Several scales exist to grade the severity of acne vulgaris, but no single technique has been universally accepted as the diagnostic standard. Cook's acne grading scale uses photographs to grade severity from 0 to 8 (0 being the least severe and 8 being the most severe). This scale was the first to use a standardized photographic protocol to assess acne severity; since its creation in 1979, the scale has undergone several revisions. The Leeds acne grading technique counts acne lesions on the face, back, and chest and categorizes them as inflammatory or non-inflammatory. Leeds scores range from 0 (least severe) to 10 (most severe) though modified scales have a maximum score of 12. The Pillsbury acne grading scale simply classifies the severity of the acne from grade 1 (least severe) to grade 4 (most severe).
Comedones (blackheads and whiteheads) must be present to diagnose acne. In their absence, an appearance similar to that of acne would suggest a different skin disorder. Microcomedones (the precursor to blackheads and whiteheads) are not visible to the naked eye when inspecting the skin and can only be seen with a microscope. There are many features that may indicate a person's acne vulgaris is sensitive to hormonal influences. Historical and physical clues that may suggest hormone-sensitive acne include onset between ages 20 and 30; worsening the week before a woman's period; acne lesions predominantly over the jawline and chin; and inflammatory/nodular acne lesions.
Hair follicles are the tiny structures that grow hair in the scalp. Sebaceous glands produce sebum. On areas where acne develops, sebaceous glands surround the hair follicles. The combination of the sebaceous glands and the hair follicles is the "pilosebaceous unit," where acne pimples and cysts develop. Sebum moisturizes hair and skin. Each hair pushes up through the skin surface along with sebum.
Genetics is thought to be the primary cause of acne in 80% of cases. The role of diet and cigarette smoking is unclear, and neither cleanliness nor exposure to sunlight appear to play a part. In both sexes, hormones called androgens appear to be part of the underlying mechanism, by causing increased production of sebum. Another frequent factor is excessive growth of the bacterium Propionibacterium acnes, which is normally present on the skin.
Acne is caused by what you eat. SOMEWHAT FALSE. This isn’t a complete lie, but studies are still being done. To date, there have only been small studies done on a few people that have shown a possible connection between acne and high carbohydrate foods like bread, pastries and sodas. There are also studies involving more people, but they relied on adults remembering their acne and eating habits when they were in high school. There just isn’t enough evidence to link any food with breakouts. Some people swear that certain foods make their acne worse. The best way to determine if a food is causing your pimples to appear is by keeping a food/skin diary. Keep track of everything you eat and assess your skin condition in the morning and evening of each day. If you find a pattern, it can’t hurt to avoid the foods that lead to more blackheads and whiteheads. Keep in mind that eating a balanced diet is always a good idea.
The earliest pathologic change is the formation of a plug (a microcomedone), which is driven primarily by excessive growth, reproduction, and accumulation of skin cells in the hair follicle. In normal skin, the skin cells that have died come up to the surface and exit the pore of the hair follicle. However, increased production of oily sebum in those with acne causes the dead skin cells to stick together. The accumulation of dead skin cell debris and oily sebum blocks the pore of the hair follicle, thus forming the microcomedone. This is further exacerbated by the biofilm created by P. acnes within the hair follicle. If the microcomedone is superficial within the hair follicle, the skin pigment melanin is exposed to air, resulting in its oxidation and dark appearance (known as a blackhead or open comedo). In contrast, if the microcomedone occurs deep within the hair follicle, this causes the formation of a whitehead (known as a closed comedo).
Properly selected face cleanser will help rid your skin of bacteria that clog pores and multiply in them. High-quality face cleaning also removes dead skin cells, making acne treatment more effective. It will take a strong but gentle remedy to remove dirt without new irritations. Therefore, you should avoid abrasive scrubs and soaps for the face, which can deprive the skin of its natural oils and may cause the painful irritation.
The costs and social impact of acne are substantial. In the United States, acne vulgaris is responsible for more than 5 million doctor visits and costs over US$2.5 billion each year in direct costs. Similarly, acne vulgaris is responsible for 3.5 million doctor visits each year in the United Kingdom. Sales for the top ten leading acne treatment brands in the US in 2015, have been reported as amounting to $352 million.
Topical and oral preparations of nicotinamide (the amide form of vitamin B3) have been suggested as alternative medical treatments. It is thought to improve acne due to its anti-inflammatory properties, its ability to suppress sebum production, and by promoting wound healing. Topical and oral preparations of zinc have similarly been proposed as effective treatments for acne; evidence to support their use for this purpose is limited. The purported efficacy of zinc is attributed to its capacity to reduce inflammation and sebum production, and inhibit P. acnes. Antihistamines may improve symptoms among those already taking isotretinoin due to their anti-inflammatory properties and their ability to suppress sebum production.
First, let’s talk about what causes acne. Pimples form when the oil and dead skin cells on your skin combine to form a plug that blocks the pores. “As the P. acnes bacteria that naturally live on skin overgrow within this plugged follicle, the area becomes inflamed and this is when you start to see papules, pustules, and cystic lesions,” RealSelf dermatologist Sejal Shah, M.D., tells SELF. The treatments ahead work to exfoliate away dead skin cells, suck up excess oil, stop inflammation, and kill the P. acnes bacteria. There are even a few treatments that target hormonal acne specifically.
Perhaps one of the most popular cleansers for combination skin care on the market today, Boscia's purifying cleansing gel works best for oily to normal skin types. It works by gently cleansing skin without stripping it and adding harsh elements to the skin. It can be a tad drying, so it works best on women with more oily spots than dry spots. You can also use it to cleanse your oily T-zone, but keep it away from your dry spots, where you may want to moisturize more.
Considerations: Ask your doctor before using any other products on your skin while using erythromycin, as it may be too irritating. This includes other prescriptions, over-the-counter medicines (including those listed on this site), and harsh or abrasive cleansers, perfumes, or make-up.2 People report burning as the most frequent side effect, and also peeling, dryness, itching, redness, and oiliness, among others.2
Meanwhile, salicylic acid, which is derived from willow tree bark, wintergreen oil or sweet birch and occurs naturally in fruits like raspberries, cantaloupe and granny smith apples, works well for most skin types. Aside from being an exfoliant that sloughs away dead skin cells and other pore-clogging impurities, it has anti-inflammatory properties that help to address inflammation, which is thought to be the primary cause of acne.
