Sulfur – Not to be confused with irritating sulfates that can be found in some cleansers and treatments, sulfur is an element that can be quite good for your skin. It removes the dead skin cells and excess oils that clog your pores. But just like any chemical ingredient, too much of it can dry your skin out. You’ll likely find it coupled with other ingredients in your cleanser, like benzoyl peroxide, salicylic acid, and resorcinol.
No single product can contain all of the necessary acne-fighting ingredients, despite what some may claim. Some chemicals shouldn’t be used together, while others work together to totally eradicate acne and all the blemishes that come with it. The best acne treatment systems can do that for you, but because everyone’s skin is unique, what works for one person may not work for another. The key lies in finding the right system, a combination of products, that work best for your unique skin.
Accutane – Accutane works to reduce the size of the skin’s oil glands and, therefore, the amount of oil that they produce. It also slows down the rate at which the skin produces cells within the pore. Your acne may get worse during the early stages of Accutane treatment, but results are promising over the long haul. Accutane – also known as isotretinoin – was discovered in 1979 and is a vitamin A derivative. It’s administered in pill form and prescribed for up to 4 ½ months or more, depending on the strength of dosage.
The third phase of the life cycle of acne is inflammation. At this point, your body’s immune system may deal with the problem, but if it doesn’t, different bacteria can move into the pore. Detecting an enemy bacterial invader, your body responds by emitting white blood cells to the infected area. In some cases, the white blood cells have a difficult time fighting the bacteria, and some die, contributing to the oily buildup and creating pus.
The main hormonal driver of oily sebum production in the skin is dihydrotestosterone. Another androgenic hormone responsible for increased sebaceous gland activity is DHEA-S. Higher amounts of DHEA-S are secreted during adrenarche (a stage of puberty), and this leads to an increase in sebum production. In a sebum-rich skin environment, the naturally occurring and largely commensal skin bacterium P. acnes readily grows and can cause inflammation within and around the follicle due to activation of the innate immune system. P. acnes triggers skin inflammation in acne by increasing the production of several pro-inflammatory chemical signals (such as IL-1α, IL-8, TNF-α, and LTB4); IL-1α is known to be essential to comedo formation.

Hormonal activity, such as occurs during menstrual cycles and puberty, may contribute to the formation of acne. During puberty, an increase in sex hormones called androgens causes the skin follicle glands to grow larger and make more oily sebum. Several hormones have been linked to acne, including the androgens testosterone, dihydrotestosterone (DHT), and dehydroepiandrosterone (DHEA); high levels of growth hormone (GH) and insulin-like growth factor 1 (IGF-1) have also been associated with worsened acne. Both androgens and IGF-1 seem to be essential for acne to occur, as acne does not develop in individuals with complete androgen insensitivity syndrome (CAIS) or Laron syndrome (insensitivity to GH, resulting in very low IGF-1 levels).
If you take a close look at the acne-fighting ingredients, you will find that no single ingredient does all the work to get rid of acne. Each one has a unique effect that plays a role in the elimination of acne, but only when they are used combined or in sequence in a system do they have ability to get rid of acne altogether. This is why it is so important to use an effective system for fight acne, not just a single product.
What's Going On: If it's big, red, and painful, you're probably experiencing cystic acne, one of the more severe types. "Cystic pimples are caused by genetics and hormonal stimulation of oil glands," says Zeichner. Not only are they large, but they're also notoriously tough to treat. They often recur in the same place, because even if you manage to get rid of one, it can keep filling up with oil again and again, like an immortal pimple.
Benzoyl peroxide is an antibacterial ingredient, and it’s very effective at killing the P. acnes bacteria that causes breakouts. But benzoyl isn’t without its downsides. The leave-on creams and cleansing treatments can dry out sensitive skin types and bleach clothing if you aren’t careful. Board-certified dermatologist Eric Meinhardt, M.D., previously told SELF that it's best to stick to formulations that have no more than 2 percent of benzoyl peroxide listed on the active ingredients chart; stronger concentrations are harder on your skin without being any tougher on bacteria.
Dapsone is not a first-line topical antibiotic due to higher cost and lack of clear superiority over other antibiotics. Topical dapsone is not recommended for use with benzoyl peroxide due to yellow-orange skin discoloration with this combination. While minocycline is shown to be an effective acne treatment, it is no longer recommended as a first-line antibiotic due to a lack of evidence that it is better than other treatments, and concerns of safety compared to other tetracyclines.
Clean your face really well using a mild face cleanser and gently pat dry. Let your skin further air dry and don’t put anything else on it. Don’t put anything else on your face like moisturizer, serum, toner, etc. After a half hour, carefully check your cheeks, chin, forehead and nose, with a hand mirror, for any shine. Also, check if your skin feels tight when you smile or make exaggerated facial expressions.
