For female adult patients, one medication that Dr. Turner often recommends is spironolactone. Many patients consider it the “miracle drug.” When used at low doses, such as 50 to 150mg per day, the androgen hormones are not cycling so rapidly, which in turn helps to relieve cystic acne, especially located on the lower cheeks, jawline and neck regions. Interestingly, spironolactone is an older drug that has been used since the ’50s, but at different doses, it acts by a completely different mechanism. Thus, it is as though it is two completely different drugs, depending on the dose, but it has the same name. The only negative aspect of spironolactone dosing at these levels is that a woman should never get pregnant while medicating with this due to ambiguity in the fetal genitalia.
This ties back into what I was saying about your face being delicate. The way you clean your underarms, or upper torso, should most definitely not be the way you clean your face. The former body parts are washed by scrubbing, usually in a vigorous manner. Your face is far too delicate for this. Rough scrubbing on your face will only irritate it and cause problems.
Stronger cases may call for prescription retinoids (such as Retin-A or Tazorac), which “are really the standard of care for most acne therapy,” says Joshua Zeichner, MD, director of cosmetic and clinical research in dermatology at Mount Sinai Hospital in New York City. Certain drugs, such as Epiduo and Ziana, combine retinoids with antibacterials and may be more effective than separate products. Because retinoids also have anti-wrinkle properties (they help stimulate collagen production), they are especially beneficial for adult acne sufferers.
What it does: Benzoyl peroxide is the only known substance which can bring oxygen under the skin surface. Since bacteria cannot survive in the presence of oxygen, when used in an adequate dosage, benzoyl peroxide eradicates 99.9% of these bacteria almost immediately. It also exerts a mild drying and peeling effect, which is thought to help prevent breakouts.1-6 Benzoyl peroxide also helps lessen inflammation.7-9 2.5% benzoyl peroxide is just as effective as higher concentrations with less side effects.6,10
Chemical peels can be used to reduce the appearance of acne scars. Mild peels include those using glycolic acid, lactic acid, salicylic acid, Jessner's solution, or a lower concentrations (20%) of trichloroacetic acid. These peels only affect the epidermal layer of the skin and can be useful in the treatment of superficial acne scars as well as skin pigmentation changes from inflammatory acne. Higher concentrations of trichloroacetic acid (30–40%) are considered to be medium-strength peels and affect skin as deep as the papillary dermis. Formulations of trichloroacetic acid concentrated to 50% or more are considered to be deep chemical peels. Medium-strength and deep-strength chemical peels are more effective for deeper atrophic scars, but are more likely to cause side effects such as skin pigmentation changes, infection, and small white superficial cysts known as milia.

Salicylic acid is a topically applied beta-hydroxy acid that stops bacteria from reproducing and has keratolytic properties. It opens obstructed skin pores and promotes shedding of epithelial skin cells. Salicylic acid is known to be less effective than retinoid therapy. Dry skin is the most commonly seen side effect with topical application, though darkening of the skin has been observed in individuals with darker skin types.
Resorcinol – Almost always found coupled with sulfur, this ingredient breaks down hardened, rough or scaly skin, while disinfecting and fighting infections5. This ingredient is often used topically to treat pain and itching stemming from small injuries like scrapes and cuts, burns, bug bites, sunburn, poison ivy and more. It’s also used to treat skin conditions like eczema, seborrhea, psoriasis, warts, corns and other disorders. It is generally found in strengths of up to 2%.

Notable adverse effects of microneedling include postinflammatory hyperpigmentation and tram track scarring (described as discrete slightly raised scars in a linear distribution similar to a tram track). The latter is thought to be primarily attributable to improper technique by the practitioner, including the use of excessive pressure or inappropriately large needles.
